Citizen Commendation / Complaint Form

The Citizen Commendation/Complaint Form allows members of the public to formally share feedback regarding interactions with the Polo Police Department. This includes both commendations for positive conduct and complaints regarding officer actions or departmental policies and procedures.

Commendations
Community members are encouraged to recognize officers or employees who demonstrate professionalism, courtesy, or outstanding service. Submitted commendations are reviewed by the officer’s supervisor and forwarded to the Administrative Division for acknowledgment and consideration.

Complaints
If a concern arises involving a department member or police procedures, citizens are encouraged to first speak with a supervisor. If the issue is not resolved to the citizen’s satisfaction, a formal complaint may be submitted using this form and directed to the Office of the Chief of Police. Complaints of a sensitive or confidential nature may also be addressed directly to the Chief of Police.

Once a written complaint is received:

  • The complainant will receive written acknowledgment.

  • The department will conduct a formal investigation.

  • Updates on the investigation status will be provided.

  • Investigations are generally completed within 30 days, unless extended due to extenuating circumstances.

Please note that knowingly submitting a false report is a violation of Illinois law and may result in prosecution.

This process is designed to promote transparency, accountability, and open communication between the Polo Police Department and the community it serves.
